Assessment and Program Approval forms

All current students of the Faculty of Fine Arts and Music may be required to apply for certain internal permissions throughout the duration of their enrolment, such as extensions.

It is the student's responsibility to ensure that they are applying for the appropriate permissions with respect to their subjects. These forms must be filled out and submitted within a timely manner with any relevant supporting documentation. Applications cannot be pre-assessed and do not guarantee an approval.

Faculty of Fine Arts and Music Assignment Extension Application

Students may be granted extensions of up to two weeks (10 working days) where the student’s capacity to complete required work by the due date has been affected by significant factors beyond their control.

Attendance Waivers: Students can apply for attendance waivers via Special Consideration.

Melbourne Conservatorium of Music Application for Excused Absences (Artistic) from Conservatorium Ensemble Activities This form is to be used to request an Excused Absences from Conservatorium Ensemble Activities due to an elite professional opportunity. If the reason for your attendance waiver relates to health, or other extenuating circumstances, then you should submit an application for Special Consideration, instead of completing this form. Except in rare circumstances applications will not be considered if submitted after Monday of Week 3 in the academic teaching semester.
Melbourne Conservatorium of Music Performance Exam Clash Notification Students must submit this form when there is a clash between two exams at the same time (not on the same day) or where there is less than an hour between scheduled exams.

Students may apply to transfer into the Composition Major from the Performance Major OR Performance (ANAM) Major at the end of the calendar year. Please refer Composition Major Handbook entry.

Practical Music StudentsPractical Examination Program ApprovalAll Recital examination students must submit a Program by the census date of each Semester to be approved by their Area Head.
Master of Music (Performance Teaching) Program Approvals (MMusPT) All Performing to Teach 1, Performing to Teach 2 and Pedagogue Recital students must submit a Program by Week 8 of each Semester to be approved by the Coordinator.
Masters EnsemblesMasters Ensemble Assessment form This form collects details of ensemble repertoire; rehearsals and performances linked to Conservatorium staff supervised ensemble or chamber music activities and is used to assess satisfactory completion of ensemble subject requirements.
