Fine Arts and Music Forms
All current students of the Faculty of Fine Arts and Music may be required to apply for certain internal permissions throughout the duration of their enrolment, such as extensions, advanced standing and subject changes.
These applications differ from those made through my.unimelb, which include Special Consideration and Leave of Absence.
It is the student's responsibility to ensure that they are applying for the appropriate permissions with respect to their subjects or enrolments. These forms must be filled out and submitted to the relevant area within a timely manner with any relevant supporting documentation. Applications cannot be pre-assessed and do not guarantee an approval.
Students cannot email their requests if a form is already available on this website.

Conservatorium Instrumental Ensemble | Piano Duo & Duet | Pre-formed duos of piano students entering Second, Third or Honours years will undertake the required five to ten-minute audition on Wednesday 2 March 2022 at 11 AM. All duos are to arrive at the same time/day and specific audition slots will be sorted at that time. Audition venues include rooms allocated for the ensemble on the fifth and sixth floors of the Ian Potter Southbank Centre. Either four-hand or two-piano repertoire may be presented. Melbourne Conservatorium of Music | Application for Excused Absences (Artistic) from Conservatorium Ensemble Activities | This form is to be used to request an Excused Absences from Conservatorium Ensemble Activities due to an elite professional opportunity. If the reason for your attendance waiver relates to health, or other extenuating circumstances, then you should submit an application for Special Consideration, instead of completing this form. Except in rare circumstances applications will not be considered if submitted after Monday of Week 3 in the academic teaching semester. |
Faculty of Fine Arts and Music | Assignment Extension Application | Students may be granted extensions of up to two weeks (10 working days) where the student’s capacity to complete required work by the due date has been affected by significant factors beyond their control. Attendance Waivers: Students can request attendance waivers for up to 2 weeks (10 working days) and must have valid reasons. Applications will not be approved without supporting documentation and must be received no later than 3 working days after the class/es were scheduled. In cases where more than two weeks of classes, (10 working days) have been missed, students will need to instead apply for Special Consideration. |
