All current students of the Faculty of Fine Arts and Music may be required to apply for certain internal permissions throughout the duration of their enrolment, such as extensions, advanced standing and subject changes.

These applications differ from those made through my.unimelb, which include Special Consideration and Leave of Absence.

It is the student's responsibility to ensure that they are applying for the appropriate permissions with respect to their subjects or enrolments. These forms must be filled out and submitted to the relevant area within a timely manner with any relevant supporting documentation. Applications cannot be pre-assessed and do not guarantee an approval.

Students cannot email their requests if a form is already available on this website.

Melbourne Conservatorium of Music Application for Excused Absences (Artistic) from Conservatorium Ensemble Activities This form is to be used to request an Excused Absences from Conservatorium Ensemble Activities due to an elite professional opportunity. If the reason for your attendance waiver relates to health, or other extenuating circumstances, then you should submit an application for Special Consideration, instead of completing this form. Except in rare circumstances applications will not be considered if submitted after Monday of Week 3 in the academic teaching semester.
Faculty of Fine Arts and Music Assignment Extension Application Students may be granted extensions of up to two weeks (10 working days) to accommodate unforeseen circumstances, where the student’s capacity to complete required work by the due date has been affected by significant factors beyond their control.
Melbourne Conservatorium of Music Waiver of Attendance This form is to be used for an attendance waiver request (beyond the number of classes students are able to miss without penalty) and should be submitted no later than 3 working days after the class that was missed. DO NOT use this form for application for leave for a full semester or longer, or if your circumstances relate to Special Consideration, these applications must be made via the student portal.
Melbourne Conservatorium of Music Exam Clash Notification Students must submit this form when there is a clash between two exams at the same time (not on the same day) or where there is less than an hour between scheduled exams.
Melbourne Conservatorium of Music – Practical Students Practical Examination Program Approval All Recital examination students must submit a Program by the census date of each Semester to be approved by their Area Head.
Melbourne Conservatorium of Music – Master of Music (Music Performance) Recital Program Approval - MMus All Recital examination students must submit a Program no less than two months prior to examination.
Melbourne Conservatorium of Music – Master of Music (Performance Teaching) Program Approvals (MMusPT) All Performing to Teach 1, Performing to Teach 2 and Pedagogue Recital students must submit a Program by Week 8 of each Semester to be approved by the Coordinator.
Melbourne Conservatorium of Music Masters Ensemble Assessment form This form collects details of ensemble repertoire; rehearsals and performances linked to Conservatorium staff supervised ensemble or chamber music activities and is used to assess satisfactory completion of ensemble subject requirements.

Chamber Music subjects – Group Application All students who wish to be considered for Chamber Music must be included in a Chamber Music application as part of a proposed chamber group.
Composition Specialisation (B-MUS PCME) Application Entry into the Composition specialisation is based on an application including a folio of works and academic results to date. Applications for 2021 close 5pm, Friday 27 November 2020
Electro-Acoustic Music (MUSI30031) Application This form is for students wishing to enrol in MUSI30031 Electo-Acoustic Music, which requires permission from the coordinator.
Minor Music Performance 1-4 Application For Performance Major students who may wish to apply for a second instrument or Composition students who may wish to continue their main instrument along with their Composition Major. (MUSI20025, MUSI20026, MUSI30050, MUSI30051)
Practical Music 1 (MUSI10220) Application For Diploma in Music students who wish to apply for enrolment in MUSI10220 Practical Music 1.
