Conservatorium Special Permission Forms

All current students of the Faculty of Fine Arts and Music may be required to apply for certain internal permissions throughout the duration of their enrolment.

These applications differ from those made through my.unimelb, which include Special Consideration and Leave of Absence.

It is the student's responsibility to ensure that they are applying for the appropriate permissions with respect to their subjects or enrolments. These forms must be filled out and submitted to the relevant area within a timely manner with any relevant supporting documentation. Applications cannot be pre-assessed and do not guarantee an approval.

Students cannot email their requests if a form is already available on this website.
